Do I need my Wisdom Teeth removed?

Neon sign black back toothbrush toothpaste 300x225 Do I need my Wisdom Teeth removed?This question is pondered by most young adults and their parents.  There is no absolute answer to this.  Most modern humans do not have enough room for 32 secondary (adult) teeth. 

Not all wisdom teeth need to be removed.  If the wisdom teeth (3rd molars) erupt, have enough room and are able to be cleaned by the patient there is no need to have them removed.

At the other end of the scale, some wisdom teeth stay fully under the gum line and covered by bone, the are called full bony impactions.  There are rare circumstances when the follicles of fully impacted lower wisdom teeth can transform into cysts or worse.  It is impossible to predict if this will happen.  My father, Luciano Vincenzo Don Giovanni Caputo, lived to be 86 with a fully impacted lower right wisdom tooth which never bothered him and never bothered me.

Most often, the upper wisdom teeth eventually erupt, many times outward and into the cheeks, causing pain and the inability to clean them.  It is best to have these removed.  As I did for a staff member, last evening.  Krista seems to be doing well today, eating pizza.  Cheryl the bookkeeper here, is shocked and amazed how “nomal Krista is today.”  Most commonly the lowers partially erupt, trap food, cause recurrent swelling, infection and cause tooth decay between and on both these third molars and the important second molars in front of them.

Upper Right Wisdom Tooth

Tooth #1 is an upper right wisdom tooth, or third molar.  Third molars are the last secondary (adult) teeth to erupt.  Their eruption time is the most variable of any tooth and they are the most commonly cogenitally missing tooth.  I do extract wisdom teeth in my Palm Harbor, Tampa FL area dental office.

To the untrained eye, this is just a bloody tooth.  By the root anatomy, a dental professional can tell this is most likely an upper molar.  The root anatomy of wisdom teeth are highly variable, ranging from one root (or three fused roots) to this one with divergent roots.  This tooth was extracted in my Palm Harbor FL dental office.  The young man was sedated, healed quickly and had no memory of the procedure.  This tooth was not badly decayed, but malpositioned and extracted at the same dental visit that the other three wisdom teeth were removed.  For more information on the cost of wisdom teeth removal in the Tampa bay area of Florida call my dentist office at 727.789.1333

Dentists today are aware that many patients are somewhat afraid of receiving dental treatment.  In my Palm Harbor FL dental office we have available multiple techniques of sedation including IV and Non-IV (oral and/or inhalation) conscious sedation.  It is possible for the dentist to effectively and SAFELY manage virtually all patients seeking dental care.  Note the roots of the upper right wisdom tooth at the end of the arrow.  Also note the nasal canula which delivers oxygen and or nitrous oxide (N2O, laughing gas.)

In most cases 32 secondary (adult) teeth do not fit in the fully grown humans mouth.  We are the only living creature to commonly have “impacted molars.”  Wisdom teeth can be removed with local anesthesia “novacaine” or with sedation combined with local anesthesia.  I am starting an IV line in this picture in my Palm Harbor FL dental office.  Various drugs will be administered through this IV line so the patient does not remember.  When done correctly, IV conscious sedation is extremely safe.

This wisdom tooth extraction  picture was taken in my Palm Harbor FL dental office.  This picture shows the lower right area after an impacted wisdom tooth was removed, before the site was sutured.  The big irregular red area is a blood clot forming.  This dental work was done while this young man was sedated / asleep.
