What is an Overdenture?
This picture shows a lower overdenture securely in place. This overall comprehensive dental restoration was completed in my Palm Harbor, Tampa FL area dentist office. Overall treatment required periodontal care, root canal therapy and dental extractions while the patient was sedated. Although no dental implants were placed here in the patients lower jaw, they (dental implants) were placed in the upper arch to achieve the same result, an overdenture. What is an Overdenture?
The answer to the question “What is an overdenture?” may sound simple enough, an overdenture is a denture that goes over teeth or dental implants. An overdenture is seen in this picture being inserted into a womans lower arch. Most remaining lower teeth were extracted in my Palm Harbor, Tampa Bay area dentist office while the patient was sedated. Four teeth were retained, root canal therapy was done on these teeth, posts with ball attachments were placed in these teeth (can be seen in this picture). The final result is a cost effective way to completely restore health, function and esthetic’s without the use of dental implants or the discomfort of a lower full denture. Picture of freshly extracted teeth
This picture may be too graphic for some people, others want to see what extracted teeth look like. I am a dentist. I treat the human mouth, I do not practice dentistry like a tooth mechanic. Not all teeth can or should be saved. Highly diseased and infected teeth can and do affect a persons overall physical health. Some considerations in making a tooth treatment decision are: overall physical health, long term intra-oral health, function, esthetics and finances. In this particular situation the correct choice for this patient was to extract all the diseased teeth, make fixed/removable upper replacement teeth, place lower dental implants and a lower dental implant supported overdenture. This is a complicated overall treatment plan, and requires every bit of the experience I have accumulated since graduation from Columbia University School of Dental and Oral Surgery in 1985. In subsequent blogs I will show various phases of this treatment. These teeth were extracted on 20June2011. Lower Right Dental Implants with Crowns
The finished crowns are seen in this picture. There are no dental implant abutments with this type of restoration. The fixation screws go thru the crowns and screw directly into the dental implants. What is an Overdenture?
An Overdenture is a hybrid type of full denture that uses either teeth or dental implants to help keep it secured. Seen in this picute of an overdenture is the side that sits on the gums and remaining teeth. The four whitish circles are the nylon attachments that snap onto the teeth (teeth in this picture rather than dental implants). In my Palm Harbor, Tampa Bay FL area dentist office I try to keep people away from full lower dentures as very few people are comfortable with them. This treatment is a cost effective way to have secure comfortable lower teeth. This is an alternative to a dental implant supported lower overdenture made possible with periodontally healthy, endodontically treated (root canal treated) teeth. Lower Right Dental Implants
The healing cover screws have been removed in this picture. The size of the dental implant is chosen by the dimension of available jawbone. These dental implants were placed in my Palm Harbor, Tampa Bay area dentist office. The types of dental implants seen in this picture are Biohorizon Single Stage and the color identifies the diameter, green 4.0 mm, blue 5.7 mm. What is TMJ?
When people say “I have TMJ”, what they usually mean is that they have some sort of problem with their TMJ. Let me first expain what the TMJ or Temporomandibular Joint is.
The temoromandibular joint makes possible the various movements of the mandible (lower jaw). It allows for the up, down, foward, backward and side to side movements. All movements of the mandible and the functioning of the teeth are closely associated with the TMJ. The structures of the TMJ and some of the information about how the function follow.
Bones: The temporomandibular joint derives its name from the two bones that form the joint, the temporal bone and the mandible. The condyloid process of the mandible and the mandibular fossa of the temporal bone form the joint. The condyle moves (articulates) inside the fossa and makes the movements of the mandible possible. It provides for the up and down movements (protrusive and retrusive), and side to side movements (lateral and rotational).
Articular Disc: The articular disc is a thin, biconcave, oval plate made of fibrous tissue, located between the mandibular fossa and the condyle of the mandible and the articular eminence of the temporal bone. The disc divides the TMJ into upper and lower cavities, each having synovial membranes which secrete synovial fluid to lubricate the joint. The edges of the disc are attached to the capsular ligament and in front it is attached to the lateral pterygoid muscle.
Ligaments: A ligament is a tough, fibrous band that connects bones. There are four ligaments that limit the extreme movement of the mandible. these are the capsular ligament, the lateral (temporomandibular) ligament, the sphenomandibular ligament and the stylomandibular ligament.
Movements of the Mandible: The articulating joint of the mandible allows for a combination of movements, such as hinge, lateral and protrusive movements. Different types of teeth perform various functions (incisors and cuspids for cutting, bicuspids and molars for crushing). The articular disc is situated between the condyle and the fossa to allow for the many different movements and functions required by different teeth. To contain the TMJ and to seal in the synovial fluid, the TMJ is encapuslated by the capsular ligament with a synovial lining. Thus, the TMJ is protected from wear and can absorb most minor blows to the jaw.

These anchors can be used to secure removable teeth or fixed, non-removable teeth as in this situation. Dental Implants with Healing Cover Screws
Completely healed dental implants with cover screws are seen in this picture. There is plenty of healthy attached gingivae (gum tissue) present. The dental implants are nicely spaced making the restorative dentistry (in this case crowns/bridge) straightfoward. I have been placing and restoring dental implants since 1989 in my Palm Harbor, Tampa Bay FL area dental office.
